Retinal Detachment Repair

A retinal detachment is a sight-threatening medical emergency where the neurosensory retina separates from the underlying retinal pigment epithelium. Surgical management involves identifying all causative retinal breaks, draining subretinal fluid, applying endolaser photocoagulation or cryopexy, and placing a temporary gas or silicone oil bubble to press the retina back into its anatomical position. Pars Plana Vitrectomy (PPV / MIVS)

Classic warning signs include a sudden shower of new floaters, bright flashes of light (photopsia), and a dark shadow or curtain progressing across your field of vision. This is a medical emergency requiring immediate attention.

Pars Plana Vitrectomy is a microsurgical procedure in which the surgeon removes the vitreous gel from inside the eye. It is performed to repair retinal detachments, clear vitreous hemorrhages, and peel macular scar tissue.

When an intraocular gas bubble is placed to seal a macular hole or reattach the retina, the gas rises upward. Face-down positioning ensures that the buoyant gas bubble exerts constant pressure against the back of the eye to facilitate healing.

If a gas bubble was injected during surgery, air travel and traveling to high altitudes are strictly prohibited until the gas completely dissolves, as pressure changes can cause severe, painful intraocular pressure spikes.

Initial physical healing occurs over 2 to 4 weeks, while visual recovery is a gradual process that can continue to improve over several months depending on the preoperative state of the macula.

The surgery is performed under local or general anesthesia with sedation, ensuring you do not feel pain during the procedure. Postoperative discomfort is typically mild and managed with prescribed eye drops and pain relievers.
